Badge Authorization Form for Students and Clinical Fellows (PDF, 1 Page) – This form is to be completed by the Personnel Security Office. Summer Students and Clinical Fellows must bring this form to the Badge Center, Building 31, Room 1B03 to pick up their ID Badge.
